A gift affidavit is a form signed by one of the spouses given the other spouse the authority to go to the department of motor vehicles in their state to transfer the title.

Immediate family members are limited to mother, father, sister, brother, son, daughter, and spouse. Complete the Affidavit of Gift of Motor Vehicles form if the vehicle is a gift.
